For the majority of those with the condition, tennis elbow gets better without any medical treatment, though rest is essential. However, if the condition causes severe and consistent pain, other treatments, including surgery, are recommended. 

Tennis elbow repair surgery can be done through open surgery, where an incision is made over the injured tendon, or using an arthroscope, which is a tiny camera on a tube. During the surgery, the surgeon scrapes away the unhealthy part of the tendon and then either attaches it to the other tendons, or repairs it with a device called a suture anchor.

Tennis elbow repair surgery is generally seen as a last resort and is usually only recommended if symptoms persist after 12 months of rest and rehabilitation. 

In addition to rest, your doctor may recommend non-surgical treatments, such as corticosteroid shots, anti-inflammatory medication, and physical therapy before surgery. Surgery may be the only answer if pain persists after these treatments, and mainly if it stops you from daily activities. Doctors also sometimes recommend repair surgery for people who suffer large tears from an acute injury in their tendons.

Patients are given anesthesia to numb the area during the procedure, whether open or arthroscopic surgery. After the surgery, it is normal to feel some discomfort. Your doctor may prescribe painkillers to help make you more comfortable.

Tennis elbow surgery is usually an outpatient procedure, meaning that you don’t have to stay the night at the hospital. However, it can take several weeks to recover, depending on how extensive your surgery was. Most people return to work between 3-12 weeks after surgery, but this varies by job. Check with your health insurance provider if your personalized coverage includes paid time off work for surgery. 

During the first 7-10 days, your movement will be quite limited, and you should keep your elbow elevated while resting. You may be required to wear a splint or a sling for the first 10-14 days. If you had open surgery, your doctor will tell you how to change your bandages and clean the wound. Avoid getting the surgical area wet in the bath or shower for the first couple of weeks or until your doctor instructs otherwise.

Returning to intense physical activity, like organized sports, takes much longer--typically about 4-6 months.

Although tennis elbow surgery is often seen as a last resort, most of the time (between 80 to 90%), patients experience complete symptom relief after surgery.

There is no exact answer as to when you will drive again because it depends on the individual and their recovery. Specifically, your doctor will not clear you to drive again until you can comfortably use your hand and fully extend the elbow. For most people, this will be within two weeks of surgery.
